Care/Case Management and Care Coordination


The PSSRU has undertaken demonstration studies and evaluations of care management in the UK, which influenced the community care reforms of the 1990s. Current work involves the mapping and evaluation of the different forms, types and models of care management for older people and those with mental health problems, that have emerged since the full implementation of the NHS and Community Care Act in 1993. Another study involves the evaluation of the effect of providing care managers with access to specialist medical assessments as part of the decision making process of entry to residential or nursing home care. This study indicates that specialist assessment at a critical moment in time can improve decision making processes and reduce the costs to the NHS.
